CROFT PATIENT PARTICIPATION GROUP
HAVING YOUR SAY IS IMPORTANT!
We would like to know how we can best deliver services to you and how you perceive our practice, staff and other NHS services
To help with this, a Patient Participation Group has been formed. Any patient can join, our aim is to have a truly representative group. We need young people, workers retirees, people with long term conditions and people from non British ethnic groups.

* There is a notice board opposite reception in the Croft where you can see all of the latest meeting information, minutes and notices. All suggestions welcome!

The practice values and learns from all feedback from our patients, families and carers. If you wish to make a complaint about any of our services you can do so in the following ways;- Directly to any of our staff who will be able to signpost you to the most appropriate person.- In writing to Karen Malecki, Practice Manager - this can be email, letter or by completing one of forms at reception.- Verbally to Karen Malecki, Practice ManagerThe NHS Complaints procedure enables the practice to manage any complaint with our patients in a way and timeframe that suits them.Please do not hesitate to contact the practice at any time with any feedback you may have.
Contact the Patient Group with your feedback, suggestion, comment or question. Please do not use
this form for complaints - these should always be directed to the Practice Manager
